The Maternity Care Coordination Program which began in October
1987 was developed to help pregnant women in North Carolina
maximize opportunities for healthy pregnancies resulting
in healthy babies. The Maternity Care Coordinators assist
clients in accessing the health care and social services
systems to ensure that outcomes which contribute to physical
and emotional health and wellbeing can be obtained. The case
management process has a primary focus on organization of
services/resources to respond to health care needs of pregnant
women.
At the Scotland County Health Department, an MCC sees all
women who have a positive pregnancy test which may have been done in the agency or at another medical facility. The social
worker screens for presumptive Medicaid eligibility and if
the worker finds a client eligible, temporary coverage for
Medicaid for pregnant women begins at this time. The women
are also referred for WIC and prenatal care if desired. Maternity
Care Coordination is initiated at this visit for all women
who desire the service. All women who receive WIC are offered
MCC services if not previously enrolled.
The Maternity Care Coordinator provides:
- Outreach-includes development of referral network and
community awareness.
- Recruitment-encouraging enrollment into the MCC program
- Assessment-determining the clients’ needs and strengths.
- Service Planning-client determined
- Coordination and referral
- Follow-up and monitoring-minimum of one contact monthly
- Education and Counseling
The Maternity Care Coordinator works with the client from
initial enrollment until the end of the month in which the
60th postpartum day occurs. In the fiscal year 2004-2005 there were 343 clients
served by the Maternity Care Coordination Program. Some
clients receive services while visiting the agency for prenatal
care and others are contacted at a time/place which is directed
by the clients’ needs.
